Full-time Position
We are an independent, 24-hour long-term care pharmacy dedicated to providing exceptional pharmaceutical care to skilled nursing and assisted living facilities across central California. Our team values clinical excellence, collaboration, and a commitment to patient safety and service.
Position Summary
We are seeking an experienced Pharmacist to join our LTC team. This is a full-time position.
The ideal candidate is a dependable, clinically strong pharmacist who thrives in a fast-paced environment, works well independently, and collaborates effectively with technicians and fellow pharmacists.
Schedule
- 8-Hour Shifts
- Rotating Weekends
Key Responsibilities
- Verify and check prescriptions for accuracy, appropriateness, and compliance with state and federal regulations
- Oversee medication preparation and dispensing for LTC facilities
- Perform sterile compounding as needed (IVs, antibiotics, etc.)
- Provide clinical support and consultation to nursing staff and prescribers
- Provide comprehensive pharmaceutical care, including medication management and patient assessment
- Collaborate with medical staff to develop and implement individualized treatment plans
- Monitor patient responses to medications, adjusting therapy as necessary for optimal outcomes
- Ensure accuracy and timeliness of medication orders and delivery
- Maintain documentation in compliance with California Board of Pharmacy requirements
- Collaborate with the pharmacy team to ensure efficient overnight workflow
Requirements
- Doctor of Pharmacy (Pharm.D.) degree or equivalent
- Active and unrestricted California Pharmacist License
- Experience as a pharmacist required (LTC experience preferred)
- Strong clinical and sterile compounding skills
- Excellent attention to detail and problem-solving ability
- Effective communication and teamwork skills
- Ability to work independently with minimal supervision
- Excellent communication skills with a focus on patient care and education.
- Ability to work collaboratively within a multidisciplinary team environment.
- Knowledge of medical terminology related to pharmacotherapy and patient care.
